Our Clinical Nurse Consultant reviews the details you provide.
We contact you
We discuss the client's needs, funding and the most suitable service.
We arrange a visit
An assessment or consultation is booked in the client's home or via telehealth.
Helpful to include
NDIS plan or goals, recent reports, current medication list and GP details. You can attach these to the form or send them later.
Service area
Nursing services are available across the Sunshine Coast and surrounding areas. Specialist Parkinson's disease consultations are also available Australia-wide via telehealth.
